Thrombose Veineuse Profonde Récidivante Révélant Un Déficit En Protéine S Chez Un Jeune Adulte : A Propos D’un Cas Observé Au CHU Joseph Ravoahangy Andrianavalona, Antananarivo, Madagascar

Abstract


La thrombophilie héréditaire constitue une cause importante mais souvent méconnue de thrombose veineuse profonde (TVP), notamment chez le sujet jeune sans facteur de risque acquis identifiable. Nous rapportons le cas d’un jeune homme de 27 ans présentant une TVP récidivante secondaire à un déficit en protéine S. Le diagnostic a été posé après une rechute survenue à l’arrêt du traitement anticoagulant. Le bilan étiologique a mis en évidence un déficit en protéine S, confirmant une thrombophilie constitutionnelle. L’évolution sous anticoagulant oral à dose préventive a été favorable sans récidive à ce jour. Ce cas illustre l’importance du dépistage des thrombophilies héréditaires devant toute thrombose idiopathique récidivante chez le sujet jeune, afin d’adapter la durée du traitement anticoagulant et prévenir les complications thromboemboliques.
