Intérêts De La Méthode De Ponseti Dans Le Traitement Des Pieds Bots Varus Equins Négliges Chez L’Enfant

Abstract


Introduction: Equine clubfoot can be a source of disability with a high risk of illiteracy, and discrimination in almost all aspects of society if not treated early. The objective of this study is to describe the epidemiological and clinical profile and to evaluate the results of Ponseti treatment of neglected clubfoot.

Materials and methods: A prospective, descriptive study of 40 equine varus clubfeet in 27 patients older than 2 years among 93 neglected PBVE patients was performed during an 08-month period from January to August 2022.

Results: The male gender was predominant with a sex ratio of 2. The average age of the patients was 4.3 years (range: 2 years and 14 years). The bilateral form predominated in 51.9% of cases. Eighty-four percent of the patients came to the Center after awareness of the possibilities of clubfoot treatment. The Pirani score at the start of treatment averaged 4.68 and was 0.5 at 5 weeks after the last cast. The number of casts before percutaneous tenotomy averaged 8.6. The cast immobilization time was 2.8 months. All patients had undergone percutaneous Achilles tenotomy. All feet were flexible and plantigrade after treatment. Three, or 11.1% of the patients required orthopedic shoes for walking.

Conclusion: The late treatment of clubfoot was related to the difficulty of geographical accessibility to the center and lack of awareness of the existence of treatment. Awareness campaigns for behavioral change must be reinforced so that the population can adopt medical management of deformities or malformations of children's limbs.
